The DeKalb Services Center provides day habilitation, day supports,
and work activity to approximately one hundred seventy (170) adults
with developmental disabilities. The facility, which opened in 1978,
is located at 2660 Osborne Road, N.E., Atlanta, Georgia. The center
is open Monday - Friday from 8:00 a.m. - 4:00 p.m. Consumers served
by the center must be 21 years old or older, live in DeKalb County,
and are in need of services due to pervasive mental retardation
or related developmental disability. Admission to the center is
voluntary.
The focus of services is on enabling individuals to acquire, develop,
enhance, improve, or maintain skills that further the consumer's
ability to function more independently in the home and community,
and enable the consumer to have a quality and meaningful day. Core
services include daily living skills training, vocational training,
behavior support, physical therapy, nursing services, recreation
and transportation. Day Habilitation (DH) and Day Support (DS) services
are offered. Consumers receiving DH often require intensive support,
and often have complex medical and physical needs. Approximately
25% of the consumers attending the Center require a wheel chair
for mobility. Day Support services are designed to enable the consumer
to more readily access community activities and services. Consumers
served under DS spend a minimum of six hours each month involved
in community activities.
The Services
Center utilizes a registered nurse and a licensed practical nurse
to provide clinic services to enrolled consumers. A licensed physical
therapist is under contract and provides oversight, direction and
direct services for the Physical Therapy program. Consumer families
are actively involved in health care decisions.
