Supports
Community Support Team
Community Support Team is an intensive community rehabilitation
and outreach service offered to consumers who need assistance in
gaining access to necessary services and support during crisis situations.
Team members work with all persons receiving the service
and go to the person in the community wherever they are located.
Interventions focus on community coping and individual living skills,
development of symptom monitoring, medication monitoring, and self
medication abilities. The
team also works with families and other caregivers in the community
to assist them in their support of the consumer. The team includes
a peer specialist who is an integral part of the team. Consumers
are usually referred to the team when they are being discharged
from the hospital or from Crisis Stabilization in need of more intensive
follow up for a transition back into the community.

Support Individual
Community
Support Individual is a service offered through all mental health
outpatient sites. This service is a part of the new rehabilitation
continuum of services. It is comprised of rehabilitative services,
environmental support, and targeted case management (which is considered
essential to assist the consumer in gaining access to necessary
services). These services are intended to restore an individual
to the best possible functional level and for maximum reduction
of symptoms of a psychiatric illness. Maintenance level consumers
may also utilize the service when needing case management types
of assistance or maintenance on medication.
