Community
Support Individual (CSI)
Community Support Individual is part of the rehabilitation continuum
of services. It is comprised of rehabilitative services, environmental
support, and targeted case management (which is considered essential
to assist the consumer in gaining access to necessary services.)
These
services are intended to restore an individual to the best possible
functional level and for maximum reduction of symptoms of a psychiatric
illness. Maintenance level consumers may also utilize the service
when needing case management types of assistance.Services
include symptom self-monitoring; coordination with family and significant
others, and with other systems of care (i.e., education, juvenile
justice, children's protective services); and services must be flexible
enough to be delivered in a variety of environments including homes,
schools, juvenile detention facilities, DFACS shelter, foster and
group homes.
Community Support Team (CST)
This
is an intensive community-based service focused on providing crisis
intervention, training in coping and independent living skills,
parenting skills, and symptom management. Services are coordinated
with other agencies, utilizing a System of Care approach that is
driven by the needs and goals identified by the family. Services
are provided in the consumer's home, school and community. A strength-based
approach is used to insure maximum use of the natural resources
available to the family. Team members are available 24/7 to assist
with crisis management. All team members are familiar with the children
and adolescents on the caseload.
